Priority violations:

• Employee did not wash hands before handling/wrapping clean silverware. Corrected.

• Date marking for ready-to-eat. Potentially hazardous food/time/temperature control for safety food prepared on-site or opened commercial container held for more than 24 hours. Previous violation.

Nonpriority violations:

• Personal items stored with and above establishment food. Personal items must be stored in such a way to prevent contamination of facility food or equipment.

• Excessive food and grease accumulation on the stove top and under the grill. Excessive grease and food accumulation on the sides and around the fryer, grill and stove.

• Excessive grease and food accumulation on the floor under the fryer, grill and stove. Accumulation of dirt and debris under and around all equipment in the kitchen area. Clean inside all refrigeration units.

• Establishment built a room inside the store room/preparation that contains a crib and other bedding items as well as some toys. The room does not meet the regulations for sleeping/living quarters. Children or unauthorized persons cannot be in food storage, food preparation or ware-washing areas. Living or sleeping quarters located on the premises of a retail food establishment such as those provided for lodging registration clerks or resident managers shall be separated from rooms and areas used for retail food establishment operations by complete partitioning and solid self-closing doors.

• Ownership change and the new owner did not contact the Department of Health to receive their new permit. Facility was operating without a valid permit.

• Employee handled bread items with bare hands. Bare hand contact with ready-to-eat foods is not allowed. Corrected.

• Wall covering is pulling away from the wall inside the walk-in. Wall around walk-in door is no longer smooth and easily cleanable.
