Priority violations:

• Automatic dishwasher is not dispensing sanitizer at all. Do not use machine until repaired. Manually wash, rinse and sanitize.

• Reusing a bottle that was used for another chemical at one time. Never reuse bottles for different chemicals. Destroy bottles as needed.

Nonpriority violations:

• Cases of food on the floor in the walk-in cooler.

• Spray nozzle at three-compartment sink is hanging below the rim of the sink.

• Leak in sink at three-compartment sink.

• Floor in walk-in is rusty and no longer in good condition. Repeat violation.

Priority violations:

• Sliced ham and turkey in upright cooler were not at 41°F or below. May have been because product was out to stock other units.

Nonpriority violations:

• Cases of food on the floor in the walk-in freezer. Food must be at least 6-inches off the floor.

• Except for a plain ring such as a wedding band, while preparing food, food workers may not wear jewelry on their arms and hands.

• Plastic ice scoop is cracked and no longer easily cleanable. Discontinue use.

• Ice build-up along the top and gaskets of the upright freezer. Wall corners in the bakery area and around the handwashing sink (dish area) are no longer in good condition. Dry wall is breaking off. Wall paint under window in bakery is peeling. Paint is peeling off the wall behind the soda machine. All repeat violations.

• Permit not posted in customer view. Repeat violation.

Priority violations:

• Food stored on ice not held at 41°F or below. Ice must be at or above the level of the food to be effective.

• Not all items date marked as needed.

• Establishment is using scented bleach for storing wiping cloths.

Nonpriority violations:

• Sanitizer concentration for wiping cloth is not strong enough. Should be 50-100 ppm.

• Condensation leak in the freezer. Do not store open boxes under area. Repair as soon as possible.

Nonpriority violations:

• Interior bin of ice machine is beginning to get rusty. Replace as needed by 2017 season.

• Repair wall next to the handwashing sink. Paint is peeling and finish sealing the corner by 2017 season.

Gentry Senior Activity & Wellness Center, 121 E. Main St.

Priority violation:

• Food received from Siloam Springs Senior Center. Lunch meat in salad was 50°F but will be served within four hours of preparation.

Priority violations:

• No soap at front handwashing sink. No dispenser.

• Burrito mix not being held at 135°F or above.

• Prep table is not holding food at 41°F or below. Unit was not plugged in all the way. Corrected. Monitor to ensure it continues to work.

Nonpriority violations:

• No thermometer in the prep table.

• Air curtain over drive-thru window is not working. Excessive flies throughout facility.

• Repair side door so it will stay closed.

• Dead or trapped insects shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests. Change glue board frequently as needed.

• Beard restraints required for food employees.

• Maintain equipment and facility in a clean condition. Develop and implement a cleaning schedule for all areas of the facility. Repeat violation.

• Accumulation of debris on equipment and ware-washing area. Repeat violation.

• Sour smell in the front area around the ice machine and ice cream machines by front handwashing sink. Repeat violation.

• Condensation leak in the copper tubing of walk-in cooler. Do not store product under leak.

• Leak in the three-compartment sink plumbing. Plug at three-comparment sink (sanitizer side) does not hold water. Soap dispenser unit at three-compartment sink is not working.

• Condensation leak in freezer.

• Permit not posted.
