Nonpriority violation:

• Faucet at the three-compartment sink is leaking.

Gentry Intermediate School, 302 W. 2nd Street

Priority violations:

• Dishwasher washed hands in prep sink, not the handwashing sink. Corrected.

• Hot box is not holding food at 135°F or above (mashed potatoes).

• Using "linen" scented bleach. Use only original bleach, no scents.

Nonpriority violations:

• Some food in freezer walk-in is not being stored 6" off the floor. Shelves are not 6" off the floor.

• Wooden shelves in freezer must be sealed so they are easily cleanable and non absorbent. Repeat violation. Correct by fall 2016 school year. Non food-contact surfaces of equipment that are exposed to splash, spillage or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, non absorbent and smooth material.

• Condensation leak in the freezer.

Priority violations:

• Handwashing sink is not accessible. Cans, cups and food in sink. Corrected.

• Food is not being held at 135°F or above.

Nonpriority violations:

• No thermometer in the three-door sandwich cooler. Repeat violation.

• Wall behind the three-compartment sink is no longer easily cleanable. Caulking is no longer in good condition or cleanable.

• Countertop on the one corner is coming unglued and is no longer in good condition and easily cleanable. Repeat violation.

• Formica is coming unglued at the edge of the three-compartment sink. Repeat violation.

Priority violations:

• Discontinue sale and use of all foods from unapproved sources.

• Interior of ice machine is not clean. Dust/debris on the inside of unit at the top of the opening.

• Crispito was only at 88°F from the fryer. Oil may not be hot enough. Recooked to 187°F in other fryer.

• Hot case is not holding food at 135°F or above. Repeat violation.

Nonpriority violations:

• Soda/beer cooler ceiling has an accumulation of dirt on it. Non food-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

• Soda/beer cooler floor is no longer in good condition. Wood floor must be sealed to ensure it is easily cleanable and non absorbent. 60 days.
