Benton County Restaurant Inspections

WESTSIDE -- Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Critical violations: Employee washed hands and put on gloves then put on hair restraint (corrected); employee re-washed and re-gloved.

Noncritical violations: Persons unnecessary to the retail food establishment operation are not allowed in the food preparation area; teacher who was not part of the food preparation staff was in the kitchen during food preparation and opened the cupboard without washing hands first and washed an item intended for personal use at the same time.

Critical violations: Employee did not wash hands after handling raw meat before changing tasks, hand washing is required between task changes. Stick butter exceeded the use-by time (destroyed).

Critical violations: Tray of raw hamburger patties was stored above cooked and ready-to-eat foods. Not all food stored on ice is being held at 41 degrees or below; ice must be at or above the level of the food to be effective (repeat violation). Not all items are date marked as needed (repeat violation).

Noncritical violations: Reusing a single-use container for pea salad. Interior of the hot chocolate machine has an accumulation of powder inside the unit. Condensation leak in two different places in the walk-in cooler; condensate was dripping on a pan of food; do not store food under the area until leak is repaired. Condensation leak in the freezer; do not store food under the area until the leak is fixed.

McDonald's

Critical violations: Employee went from grill to sandwich line without washing hands; this is a task change, hand washing is required between task changes. Back hand washing sink is not shutting off, so it is turned completely off and employees have to manually turn on and off; be sure to shut off at button with a paper towel until the unit can be repaired properly.

Noncritical violations: Broken thermometer in the milk cooler. Food workers shall wear hair restraints, including managers, when they are doing food preparation. Spilled product inside the milk cooler.

WACD/Gentry Early Learning Academy

185 Rebecca St., Gentry

Critical violations: None

Noncritical violations: Condensation leak in the walk-in cooler and open boxes of food were stored under dripping water; do not store food under area until repaired (boxes moved). Floor, door frame and threshold of walk-in cooler are no longer in good condition, floor is rusty, threshold and door frame are falling apart, exposing insulation; this is an ongoing violation and needs to be corrected properly and permanently. Paint and wall behind the dish machine is damaged; paint is starting to bubble, indicating a water leak. Leak in the sink drain of the three-compartment sink; this is a repeat violation and must be corrected.

Nov. 16

Long John Silver's

525 U.S. 412 West, Siloam Springs

Critical violations: Employee did not wash hands between task change and glove change. Dishes in sanitizer, but the sanitizer was not strong enough; changed the sink water to correct the problem. Interior of prep tables in kitchen and in front have an accumulation of debris on the top and lower portions of the units. Interior of the ice machine is not clean. Not all foods are being held at 41 degrees or below. Cole slaw on top of unit in pan is 43 degrees; portioned cole slaw in bottom unit is at 53 degrees on the top and 45 degrees in the bottom (destroyed or removed as needed). Hot dogs are not date marked.

Noncritical violations: Some containers are cracked, broken or heat blistered; containers are no longer in good condition and need to be replaced. Area behind the ice machine is not clean; wall behind the three-compartment sink is not clean. Excessive ice build-up on the floor in the walk-in freezer and around the door, ice starting to get under the walk-in cooler floor causing it to buckle. Tile is missing at the floor wall junction around the mop sink and the opposite wall, allowing for standing stagnant water and cannot be easily cleaned; area around the mop sink is not in good condition. Sprayer at the three-compartment sink will not shut off at all; there is a leak in the faucet of the three-compartment sink.
