Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Sept. 29

Dollar General

Critical violations: None

Noncritical violations: Clean interior of egg/deli meat cooler; clean interior and exterior of milk coolers in the back store room, product spilled and excessive build-up on the bottom and shelves; clean shelves of milk coolers. Condensation leak in storeroom freezer; do not store product under leak; condensation tube is disconnected.

Sept. 30

Siloam Springs Middle School

600 S. Dogwood St., Siloam Springs

Critical violations: None

Noncritical violations: Condensation leak in walk-in freezer, extra frost build up on the freezer walls; ensure insulation is enough and seals are in good condition; manager said unit was repaired during summer of 2016 but for some reason the issue still exists; contact a professional company or the original installer to see what the underlying cause may be.

Siloam Springs Intermediate School

Critical violations: None

Noncritical violations: Small leak in the drain and faucet at three-compartment sink sanitize side.

Oct. 6

Mazzio's Pizza

906 W. Kenwood St., Siloam Springs

Critical violations: Items on the dressing side of the salad bar are not holding at 41 degrees or below; move ham to the side of the salad bar that is holding food at proper temperature (repeat violation).

Noncritical violations: No thermometer in salad cooler. Except for a plain ring, such as a wedding band, while preparing food, food workers may not wear jewelry on their arms and hands. Effective hair restraints are required for food employees, including managers, when preparing food. No valid test strips for quaternary ammonia in three-compartment sink; test strips are old and discolored. Surfaces have an accumulation of grease and dirt; detail clean the prep table interior, gaskets, ceiling tiles and crevices of equipment; detail clean all the walls (repeat violation). Condensation leak in the walk-in freezer.
