Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Eureka Pizza

202 U.S. 412 West, Siloam Springs

Critical violations: Prep table is not holding food at 41 degrees or below; interior thermometer is reading 50 degrees, walk-in cooler is holding food at 41.8 degrees, interior thermometer is reading 41 degrees.

Noncritical violations: Food preparation employees do not have effective beard restraints. Tiles in dish room around three-compartment sink need to be re-grouted.

Hawg City Grill

Critical violations: Items are not date marked as required (repeat violation). Reusing spray bottles for bleach water that contained another product; do not reuse bottles for other products other than what was originally in the bottle, bottle was emptied and destroyed. Using splashless bleach for food contact surfaces; use only chlorine products that are approved for food contact surfaces.

Noncritical violations: A bag of onions was stored on the floor in the back storage area; food must be stored at least six inches off the floor. Cases of food were stored on the floor in the walk-in cooler and walk-in freezer; food must be stored at least six inches off the floor. Food preparation employees are not wearing effective beard restraints. Interior of the kegerator has some liquid spilled on the bottom; clean frequently to prevent the accumulation of food and liquid. There is no working light in the walk-in freezer (repeat violation).

Critical violations: No sanitizer being dispensed at three-compartment sink; check sanitizer concentration every day. No sanitizer being dispensed into dishwasher; bottle is empty, check sanitizer concentration every day. Date marking is inconsistent; some items marked, others not; items not used in 24 hours must have a date mark.

Noncritical violations: Cinnamon rolls covered with garbage-type bag; only use food grade bags. Reusing some single-use containers for food storage.

Critical violations: Meat side of sandwich station is not holding all food at 41 degrees, readjusted the unit.

Noncritical violations: None

McDonald's

Critical violations: Burrito and gravy in breakfast cooler were at 43 degrees, air flow may have been restricted by too many items blocking air from getting to the bottom of the unit; gravy was just prepared this morning.

Noncritical violations: Ice scoop stored on top of ice machine; store it in or on something that can be washed and sanitized.

Critical violations: Items checked on salad bar are not holding at 41 degrees or below, hummus and lettuce at 45 degrees, boiled egg at 44 degrees. Salad bar temperature log not completed at the end of service.

Noncritical violations: Multiple water leaks from ice machine drain. Paint peeling on part of dish room wall, no longer in good condition; must repair so that wall can be cleaned. Condensation build-up in walk-in freezer; check seals along ceilings, walls, ceiling insulation and seals in ceiling.
