Critical violations are those factors leading to food-borne illness and must be corrected immediately. Noncritical violations relate to maintenance of food operations and cleanliness.

467 Gentry Blvd., Gentry Critical violations: Sanitizer at the three-compartment sink is not dispensing sanitizer. Noncritical violations: Ice cream mix spilled/splashed onto the equipment next to the ice cream machine. Mix visible on the floor next to the ice cream machine. Interior of the milk cooler has standing water-milk in the bottom. Ice buildup on the frame and door of the walk-in freezer on the walk-in cooler side.

Sleepy Hollow Store Bakery

12761 Arkansas 59 South, Gentry Critical violations: Not all items in the bakery refrigerator are date marked as needed. Noncritical violations: Ingredient labels must be placed on each packaged container. The wooden legs and side panels of the bakery table are no longer smooth and easily cleanable. Underside of wooden shelves in the walk-in cooler are not sealed. Shelves are visibly dirty on the underside. Bakery table legs and side panels are visibly dirty. Bulk bin lids in both the bakery and the bagging area are visibly dirty. Cart used to transport rolls from the bakery to the front has an accumulation of glaze/food. Ice buildup on the exterior of the walk-in freezer around the door.

136 S. Gentry Blvd., Gentry Critical violations: Handwashing sink in the preparation area has items in it. Sink is not accessible. Open package of sliced ham is not date marked as needed. Possible mouse droppings under the rolling table and the roll rack next to the table. Possible droppings on some of the storage containers. Area was not accessible to take a sample. Noncritical violations: Observed ants in the food preparation area around boxes on the floor next to the frosting station. Using cardboard boxes as shelves hanging on the wall above the chest freezer in the three-compartment sink area. Wooden shelf in the reach-in cooler. The side of the shelf is exposed wood. Walls behind the glazing station and the wall behind the colored glazing station are visibly dirty. Interior of the cappuccino machine has an accumulation of powdered product. Exterior doors/handles of the reach-in cooler and the two-door cooler are visibly dirty. Permit posted expired 3/31/2019.
