Nonpriority violations:

• Clean around the air vents and the ceiling in the food preparation room.

• Condensation and ice build-up around the door and condensation unit. Check seals and insulation.

• Permit posted but expired 5/31/15.

The Station, Junction of Highways 59 and 12

Nonpriority violations:

• Some foods in the hot-holding unit were not at 135°F or above, unit adjusted.

• Food employee not wearing effective hair restraints during food preparation.

• Storing wrapped biscuits in cardboard boxes. Food must be stored in containers that can be washed and sanitized regularly.

• Wooden floor in the drink walk-in cooler is no longer in good condition. Paint has peeled off surface exposing raw wood. Unfinished wood is not an approved surface for food storage. Repair as needed to bring the storage area up to current code. Surfaces must be smooth, easily cleanable, non absorbent and durable.

• Paint is peeling off the surface of the frame surrounding the condensation unit in food walk-in cooler. Surface is no longer smooth, easily cleanable, non absorbent and durable.

• One chest freezer has duct tape holding the insulation panel in place. Duct tape is not an approved repair material. One chest freezer gasket is torn.

• Door frame between the drink and food walk-in coolers is falling away from the frame or missing, exposing drywall and wood. Other areas look like possible mold is growing on the surface.

• Condensation leak in the drink walk-in cooler causing standing water to accumulate on the floor.

• Moldy shelves in the food walk-in cooler.

• Ceiling in the food walk-in cooler is falling. Water/condensation leaks in the cooler causing standing water to accumulate on the floor. Food must be stored to prevent water from dripping on food.

• Lights in food preparation/storage area, above the ice machine and food display cases are not shielded.

Gentry Intermediate School, 302 West 2nd Street

Nonpriority violations:

• Food must be stored at least 6" off the floor. The wooden shelves in the walk-in are not 6" off the floor.

• Metal shelves in walk-in are rusty and no longer easily cleanable. The wooden shelves must be sealed so they are non-absorbent and easily cleanable.

Nonfood-contact surfaces of equipment that are exposed to splash, spillage or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, non absorbent and smooth material.

Gentry High School, 1155 Pioneer Lane

Priority violations:

• Observed two different employees not re-washing their hands after leaving the restroom. They said they were not aware they needed to wash again even though they washed their hands in the restroom, corrected.

• Employee handled buns with bare hands, corrected.

• Monitor automatic dishwasher. The machine is not sanitizing properly every cycle. Thermolabel was run through once and indicated the machine was not getting hot enough. A second label was run through and it indicated the machine was getting hot enough. The wash cycle may not be getting hot enough.

• Silver hot box is not holding food at 135°F or above. Employees say that it does not work properly. Repair or replace as needed. Do not use unit until proper temperatures can be maintained.

Nonpriority violations:

• The thermometer in the silver hot box is not working properly. Use an alternative thermometer for inside the hot box.

• Ambient air and water temperature measuring devices should be accurate within two degrees of the intended range. Suggest placing a back-up thermometer in the walk-in cooler near the door. The unit's thermometer was reading 32°F but food that was checked was 40°F.

• Some shelves in the salad cooler and walk-in are rusty and no longer easily cleanable, re-coat or replace as needed.

Sonic, 316 South Gentry Boulevard

Priority violation:

• Employee went from handling raw meat to another task without washing hands, corrected.

Nonpriority violations:

• Maintain handwashing sinks in a clean condition.

• Water standing in the bottom of the prep table, repair.

• Maintain equipment and facility in a clean condition. Implement the cleaning schedule that was recently developed. Accumulation of debris on equipment and ware-washing area. Sour smell in the front area around the ice machine and ice cream machines.
